If you believe someone is experiencing a medical emergency, contact the appropriate local emergency service or go to the nearest emergency facility immediately.
General information only
Website content is provided to explain Synapse Digital Care’s coordination services. It is not a medical diagnosis, prescription, treatment recommendation, or substitute for an examination by a qualified healthcare professional.
Telemedicine
A telemedicine consultation may not be suitable for every condition. The involved healthcare professional determines the appropriate scope, identity and consent requirements, limitations, need for an in-person examination, and clinical next steps. Technology problems, incomplete information, and the lack of a physical examination may limit what can safely be assessed remotely.
Care coordination
Synapse may help organise access, communication, referrals, movement, and practical logistics. Unless expressly agreed and lawfully provided by an appropriately qualified professional, coordination activity is not itself clinical care.
Patient transport and logistics
General transport coordination is not an ambulance or emergency response service unless that capability is expressly identified and agreed through an appropriately equipped provider. A patient’s clinical stability and transport needs should be assessed by the responsible healthcare professional. Transport and logistics providers may apply their own safety rules and acceptance criteria.
Referrals and outcomes
A referral enquiry does not guarantee provider acceptance, admission, a specific treatment, appointment timing, recovery, travel permission, or any particular outcome. Circumstances and professional decisions vary for each patient.
Independent clinical judgement
A referral or provider suggestion is intended to support access, not to direct a clinical decision or guarantee a provider’s quality, availability, fee, or outcome. The responsible healthcare professional and patient must make clinical and treatment decisions using the information available to them.
Submitting information
A general enquiry does not create a clinician-patient relationship. Do not send detailed medical records, identity documents, or payment information through the public form. Follow the instructions provided after initial contact when additional information is appropriate.
Pricing and financial decisions
Pricing is provided after assessment of the requested service. A coordination estimate is not a guarantee of the final charges imposed by hospitals, clinicians, laboratories, transport providers, airlines, accommodation providers, insurers, or government authorities.
